Date: April 16, 2007 Time: 8 AM-12 PM and 1 PM-6 PM
Date: April 17, 2007 Time:8 AM-12 PM and 1 PM-3 PM
The child must be 3 or 4 years old on or before September 1, 2007 in order to register for Head Start.
Please bring the following information to register your child:
1. Shot Records
2. Official State Birth Certificate
3. Proof of Income (2006 1040 tax form)
4. Proof of Residence (utility bill with physical address)
5. Child's Social Security Card
6. Proof of Federal Assistance (Medicaid, TANF,WIC)
7. Photo ID of parent/guardian registering
